Generate a Medical History Summary
What: Medical History Summary is an AI generator in the Clinical Assistant panel that compiles a patient's full medical journey into a professional, chronological narrative.
Why: It turns the raw, chronological patient timeline into a structured document another doctor, institution, or insurance company can read and understand quickly.
When: For patient transfers, insurance requests, specialist onboarding, or any time you need the full clinical picture in one document.
Steps
1. Open the patient's file
Location: Calendar, Waiting Room, or patient search
Open the patient's file from your schedule, the Waiting Room, or the search bar. You can generate a Medical History Summary from the Patient Overview at any time, or from the Plan tab's 'Letters' section while a consultation is open, both use the same generator.
2. Open the Clinical Assistant panel
Location: Right sidebar
Click the Lightbulb icon in the right-hand sidebar. Under 'Generate documents', click 'Medical History Summary'.
3. Scope the summary (optional)
Location: Medical History Summary dialog
A dialog titled 'Medical History Summary For [Patient name]' opens, with tabs across the top for all five document types. In the field labelled 'Note any time period, conditions, or focus areas for the medical history', narrow the scope if you need to, otherwise leave it blank for a summary of the complete timeline. For example:
- "Create a medical history summary for the past 3 years only."
- "Summarize all consultations related to the patient's cardiac health."
- "Generate a brief summary of every consultation since 2006 for an insurance company request."
4. Click 'Generate'
Location: Medical History Summary dialog
Click the orange 'Generate' button. The AI organises every diagnosis, treatment, and significant event into a professional, chronological document.
Note: This generator reviews more of the timeline than the other document types, so it can take noticeably longer to finish, give it a minute or so.
5. Review and share the summary
Location: Medical History Summary dialog
Read the summary, organised under a Patient Overview section and a dated, condition-by-condition Medical History section. If it needs changes, add a follow-up instruction (e.g. "Please add more detail about their 2022 hospitalization") and click 'Regenerate'. When you're satisfied:
- 'Copy to Letterhead', copy the document onto the practice letterhead template.
- 'Download', save it as a document to attach or print.
You should see: the summary is also saved automatically to the patient's timeline for future reference, you don't need to save it separately.
Advanced AI feature: The Clinical Assistant panel requires the Advanced AI subscription to be enabled on your account. If the panel doesn't load or the lightbulb icon is missing, your practice's Business Consultant can help activate it.
Frequently asked
Q: Can I generate a summary for a specific period, or does it always pull the entire history?
You can specify a timeframe. In the instruction box, type your requirement, such as "Generate a medical history for the past 3 years only," and the AI will tailor the summary to that period. If you leave it blank, it will generate a summary of the complete timeline.
Q: How is this different from just printing out the patient's timeline?
The patient timeline is a raw, chronological list of every event. The AI Medical History Summary synthesises this raw data into a structured, professional narrative. It logically groups related events and presents the patient's story in a format that's much easier for another doctor, institution, or insurance company to read and understand quickly.
Q: Can I be sure the summary includes everything important from the patient's record?
The AI is designed to be comprehensive and reviews the entire patient timeline to compile the summary, making it excellent at finding all recorded events. It's still best practice to give the final document a quick review to ensure it meets the specific clinical context and requirements before sharing it.
Q: Can I edit the summary after it's generated?
Yes. The AI provides a thorough first draft. You always have full control to manually edit the text or use the instruction box to ask the AI to regenerate the summary with a different emphasis or additional details before finalising it.
