Condition Tracker

The condition tracker has been added to Healthbridge Clinical to allow a practitioner to keep track of the high-risk patients, this allows these high-risk patients to be added to a list for further monitoring and closer inspection.

Condition Tracker

What: A practice-wide register of patients grouped by diagnosis code, with risk level, test results, and notes, separate from any single patient's file.

Why: Gives you a working list of every patient with a given condition (e.g. a chronic disease or an outbreak like COVID-19) in one place, so you can follow up and message them as a group.

When: When managing a cohort of patients with a shared diagnosis, chronic conditions, screening programmes, or public health tracking.

1. Open the Condition Tracker

Location: Home dashboard › Condition tracker tab

From the home dashboard, click the 'Condition tracker' tab, it sits alongside 'My schedule', 'Visit history', and 'Pathology centre', not inside any individual patient's file. By default it's filtered to Diagnosis Codes U07.1 and U07.2 (COVID-19).

To track a different condition, remove those codes and start typing in 'Enter Diagnosis…', optionally narrow by age range or gender, then click 'Apply'.

2. Add a patient

Location: Condition tracker › Add patient

Click 'Add patient' and search by name in the dialog that appears. Select the matching result to move on to their tracking details.

Note: Adding a patient here, via 'Add patient', is the reliable method, there's no condition-tracker icon in the consultation's 'Plan' tab.

3. Capture their tracking details

Location: Add patient › detail dialog

The detail dialog adapts to the tracked condition, for the default COVID-19 view it shows Risk (High/Medium/Low), test type (Rapid Antigen or RT-PCR), Test Status, Test date, Result, and a Notes field. Fill in what's required and click 'Add patient' to save them to the tracker.

Frequently asked questions

Q: What can I do with a patient once they're on the tracker?

Each row in the tracker's table has action icons to send an SMS update, edit their risk level and results, or delete them from the tracker entirely.

Q: Can I track more than one condition at a time?

Yes, add multiple diagnosis codes to the 'Diagnosis Codes' field before clicking 'Apply' to see patients matching any of them in one combined list.

Q: Does adding a patient here affect their patient file?

The tracker is a separate, practice-level register for follow-up and reporting, it doesn't add anything to the patient's own clinical notes or timeline.
